Summit Fitness Training's goals are to work with you so that you are healthier and more fit. We enjoy working with people of all health and fitness levels.

However, to ensure the safety of all participants, several health conditions require a doctor's permission and/or clearance before you can safely work out.

You will need a Medical Clearance and Physician's Consent Form completed by your doctor and returned to Summit Fitness Training before your first work out if either or both of the following conditions apply to you:

If you have one or more of the following physical conditions:

  1. Diabetes
  2. Asthma
  3. Chronic Obstructive Pulmonary Disease
  4. Abnormal resting EKG
  5. Pregnant
  6. Any other condition that Summit Fitness Training in their sole discretion may deem to present an unreasonable risk to your health, were you to participate in a fitness evaluation or program.

If you have two or more of any of the following coronary artery disease risk factors:

  1. Hypertension (>=140/90mm Hg) confirmed by measurements on at least two separate occasions or on antihypertensive medication
  2. Hypercholesterolemia: total serum cholesterol >200 mg/dL, or HDL < 40mg/dL or on lipid lowering medication. (If LDL available then use LDL >130 mg/dL and if HDL> 60mg/dL then subtract one risk factor).
  3. Family history of heart disease or sudden death prior to age 55 in father, brother or son or before 65 years of age in mother or sister or daughter.
  4. Smoking or those who quit within the previous six months
  5. Fasting glucose >= 110 mg/dL confirmed by measurements on at least two separate occasions
  6. Body Mass Index >= 30kg/sq m (subject to clinical judgement)
  7. Sedentary lifestyle, i.e., persons not participating in a regular exercise program or accumulating 30 minutes or more of moderate physical activity on most days of the week (current recommendation from US Surgeon General)

Print the Medical Clearance and Physician's Consent Form and give it to your doctor before you work out, but only if conditions described on the left apply to you.

 

Your doctor can give the form back to you or send it directly to Summit Fitness Training.
